The "Category Totals and Ranking Summary" is used to compile facility evaulations for individual sites. Percentages are automatically calculated using the Office of Public School Construction (OPSC) formula noted on the page.

School Facility Conditions Evaluation

Provided on this page are customized versions of the State Allocation Board's Facility Inspection Tool (FIT) forms. The PDF documents have been modified to allow electronic text entry and to automatically calculate category totals and ranking.

Adobe Acrobat Pro or Adobe Reader 8 (or Later) Required: Because of the scripts and calculations involved in creating these interactive documents, Adobe Acrobat Professional 8 (commercial software) or Adobe Reader 8 (free software available for download) or later are required to complete these electronic forms.

Please double-check to make sure these files open in the latest version of Acrobat Professional or Adobe Reader. If you are unsure what software you have installed on your computer, please contact your school/district technical support staff for assistance.


Form Completion Tips

After entering data into one of the forms, save the PDF to your computer (File then Save As) for future reference.

Use a logical file name when saving files (something like: "2007 FIT Evaluation Page 1.pdf").

Be sure to enter a value in the "Total Number of Areas Evaluated" field.

If there aren't enough "Area" rows available on the Evaluation Detail Sheet for the areas you have evaluated, complete and save the first sheet (refer to file naming suggestions above); then start a new document. Be sure to enter the appropriate page numbers.

To create a new document, it may be easier to duplicate a PDF you have already saved to your computer.

Mac OS: Right-click or control-click the original saved file and choose "Duplicate."

Windows: Open "My Documents" and navigate to the location of the original saved file. Select the file. In the File and Folder Tasks sidebar, choose "Copy this file." In the "Copy Items" window, navigate to the drive or folder where you wish to save the duplicate file and choose "Copy."
